Service Warranty and Agreement

I, Steve Axtell C.H. agree to work with you____________to make reasonable progress on_______________________________________________________

for the sum of $__________ (paid over time) as follows________________________________________

or for the discounted rate of $____________ (paid in full).

I will work with you for as long as necessary to accomplish your desired outcome as stated above,  provided that you do assigned practice, exercises, and listen to the audios that I give you as recommended. Success requires teamwork. We must both give it 100%. If, during our work together, you discover other issues that you want to include, we can re-negotiate our agreement.

If by the end of our second session, you decide that this approach is not appropriate for you, simply tell me, and I will gladly give you a full refund of all payment that you have made. You are free to keep any audio recordings or other materials that I have given you.
